about the health care center
Cedar Hill Health Care Center is a handsomely built, state-of-the-art, skilled nursing center. We can accommodate up to 40 residents in private and semi-private rooms. We have a professional, multi-disciplinary staff and an on-site staff development coordinator. Residents come to Cedar Hill for long-term care, rehabilitation, respite, and hospice care. We work hard to provide each resident with personalized care.
High Satisfaction, Quality Staff
In a 2006 survey of Cedar Hill Health Care's residents and families by Press-Ganey Associates, 93 percent of those who took the survey said they were likely to recommend Cedar Hill to others. That ranked us in the 95 percentile on that question compared to peers nationwide and in New England.
Our focus with residents who come to live here is to maintain the highest level of independence and quality of life.
Residents and their families are a key part of our care team. We invite residents and families to regularly scheduled, multi-disciplinary meetings where we review and assess our plan of care for that resident. We discuss together if we are meeting the resident's needs and wishes and furthering their well-being. We know this is THEIR home.
Rehabilitation Services
Our short-term rehabilitative residents work intensively with our nurses and aides, and our on-site, seven-day-per-week physical, occupational, and speech therapists to regain the skills to return home or transition to a more independent setting.
Respite Services
Cedar Hill also provides respite care to older adults whom family or at-home aides are caring for at home. We can schedule respite care for as long as the caregiver needs. Our respite residents enjoy all the services and amenities of Cedar Hill. Please see our page on our respite services.
Hospice
We provide hospice services for terminally ill residents. We call this our Angel Program. Our staff and our pastoral counselor work as a team with family and significant others. Our goal with our Angel program is to respect the resident's wishes, emphasizing his or her comfort, dignity, and spiritual needs during this time of life.
Medical Care
Each resident is under the care of a physician who is responsible for the medical plan of care. Our staff - including registered nurses (RN), licensed practical nurses (LPN), and licensed nursing assistant (LNA) - provide around-the-clock care and supervision. We work with consultant staff specialists for services such as physical therapy, speech therapy, dental services, laboratory and x-ray as needed.
Cedar Hill has a medical director who works with our staff and other doctors. Medical doctors are available to serve our residents from several area physician practice
groups.
We work with four hospitals: Dartmouth-Hitchcock Medical Center, Mt. Ascutney Hospital, Valley Regional Hospital and Springfield Hospital.
